Day Crisis Care Center Case Manager

Job Summary
Case Manager at Crisis Care Center responsible for coordinating discharge planning, warm handoffs, and linking clients to community resources after crisis stabilization. Duties include assessing client needs, developing individualized service plans, implementing discharge goals, serving as liaison between clients and service providers, maintaining detailed client records, providing transportation when needed, and preparing documentation for daily/weekly reports and treatment team meetings. Requires autonomous work, strong communication with a multi-disciplinary team, and familiarity with crisis intervention and psychotropic medications. Location is Birmingham, AL; standard daytime hours Monday through Friday.
Required Qualifications
- Bachelor's degree in social work, psychology, counseling or other human services field
- Experience working with individuals with serious mental illness and/or substance use issues
- Experience working in case management or outreach (preferred)
- Valid Alabama driver's license and a suitable automobile to permit the required travel
- Must be insurable and maintain insurability under automobile insurance standards
- Must show and maintain car insurance equal to or in excess of Alabama standards
- Knowledge of seriously mentally ill and substance use population
- Ability to work autonomously
- Knowledge of the community resources and applicable rules/regulations
- Ability to communicate effectively with a multi-disciplinary team
- Trained in or willing to be trained in crisis intervention, Therapeutic Options
- Knowledge of casework principles and methods related to case management
- Working knowledge of clinical interviewing techniques and principles
- Ability and willingness to document activities and maintain records in the electronic health record
- Knowledge of accessing medication assistance for clients with no resources
