Coordinate individualized activity plans that align with participants' interests, goals, and strengths, covering recreational activities, volunteer experiences, educational programs, employment exploration, wellness initiatives, and community events. Foster strong relationships with community partners to provide person-centered support that empowers individuals to maximize independence and achieve meaningful community inclusion. Maintain reliable transportation and utilize computer skills for data entry and communication while ensuring daily safety and engagement for participants across Massachusetts, Connecticut, New York, Rhode Island, and Oklahoma.
Required Qualifications
- High School/GED
- Must have a valid driver's license, good driving record and a reliable vehicle
- Ability to use computers to include Microsoft Word, data entry, email and internet essential
