Job Summary
Coordinate virtual visits, onboard patients to digital platforms, monitor remote exercise and symptom data, and ensure accurate documentation in the EHR. Collaborate closely with PT, OT, and SLP teams to streamline workflows and improve access. Manage delivery of outpatient rehab through telehealth and connected care tools while maintaining patient-centered, evidence-based care standards.
