Job Summary
Coordinate virtual visits, onboard patients to digital platforms, and monitor remote exercise and symptom data while ensuring accurate documentation in the EHR. Collaborate closely with PT, OT, and SLP teams to streamline workflows, improve access, and enhance outcomes through telehealth and connected care tools. Support the delivery of outpatient rehab via digital health initiatives within a collaborative, growth-focused environment.
