Job Summary
Direct care and coordination for individuals in Community Residential Rehabilitation level 3 services, including overseeing progress toward measurable goals, implementing and reviewing the Individual Service Plan (ISP), conducting assessments for individualized plans, and coordinating with primary care and community partners. Emphasizes behavioral health, physical health, crisis intervention, and documentation of client progress; requires strong written and oral communication and supervisory experience. Location: Milledgeville, GA; Schedule: Monday–Friday, 8:30am–5:00pm. Education includes a high school diploma or GED; bachelor's degree preferred; supervisory experience required.
Required Qualifications
- High School diploma or GED (Required)
- Bachelor's degree in a related field (e.g., psychology, sociology, or similar) from an accredited college or university. (Preferred)
- Minimum of two (2) year of supervisory experience.
