Job Summary
Clinical Service Coordinator for the Adult Outpatient team responsible for biopsychosocial assessments, treatment planning, care coordination, and discharge planning for adults with mental health and co-occurring disorders. Roles include conducting assessments, developing and monitoring treatment/discharge plans, coordinating referrals and transitions, maintaining clinical documentation, and collaborating with treatment teams and community partners. Telehealth may be utilized; duties emphasize documentation, reporting, and maintaining a minimum productivity standard (60 billable hours per month). Requires a Master’s degree and active pursuit of Georgia licensure (LMSW/LCSW, APC/LPC, LAMFT/LMFT) with eligibility for licensure.
Required Qualifications
- Master’s degree in social work, psychology, counseling, or a related behavioral health field
- Eligible for Georgia licensure as LMSW/LCSW, APC/LPC, LAMFT/LMFT
- Must actively pursue professional licensure upon hire
- Experience conducting biopsychosocial assessments and treatment planning (implied in duties)
- Experience coordinating care and discharge planning in outpatient behavioral health settings
