Job Summary
Lead the CTI model implementation and supervise a multidisciplinary team (case managers, LPN, and peer specialists) to support individuals transitioning from hospital to community care. Build partnerships with community providers, run regular team meetings, ensure accurate, timely regulatory-compliant documentation, track program outcomes, and drive service improvements. Provide coaching and professional development for staff, report program performance to leadership, and advocate for clients within the agency and broader community. Salary range $68,000–$71,000 plus a $5,000 sign-on bonus; comprehensive benefits included.
Required Qualifications
- LMSW, LCSW, LMHC, or related clinical license
- At least 3 years of supervisory experience
- Valid driver’s license and reliable transportation
- Ability to work occasional evenings or weekends as needed
- Comfort with Microsoft Office tools
