Job Summary
Lead the CTI (Critical Time Intervention) program implementation and supervise a multidisciplinary team (including case managers, LPN, and peer specialists) to guide clients through pivotal transitions from hospitalization to community care. Build partnerships with community providers, run regular team meetings, ensure accurate and timely documentation compliant with regulatory standards, track program outcomes to drive service improvements, and support staff growth through coaching and professional development. Advocate for clients’ needs within the agency and broader community, and report program performance to leadership with clarity and accountability.
Required Qualifications
- LMSW, LCSW, LMHC, or related clinical license
- At least 3 years of supervisory experience
- Valid driver’s license and reliable transportation
- Ability to work occasional evenings or weekends as needed
- Comfort with Microsoft Office tools
