Job Summary
CSF Parent Navigator supports and coordinates services for families in Region 5 CSF. Responsibilities include co-managing a caseload of ~12 families with CSF workers, using the WRAP process to build care plans, conducting weekly contacts and joint home/community visits (including evenings/weekends), coordinating Family Team Meetings, linking families to community supports and resources, advocating in community meetings, and maintaining secure, confidential family records. Requires a college degree in a human-service field (AA/AS or BA/BS); bilingual may be required; valid CT driver’s license and own vehicle for home visits (agency vehicle may be used). Must work independently and as part of a team and participate in mandatory trainings; duties performed under Wellmore guidelines.
Required Qualifications
- college degree (AA/AS or BA/BS) in a human service field (e.g., social work, marriage and family therapy, counseling, or psychology)
- 3 years’ experience working with families (preferred)
- bi-lingual (Spanish-English) may be required
- valid Connecticut driver’s license, personal transportation and valid auto insurance
- ability to maintain confidentiality
- CPR and first-aid
