Job Summary
Provide 1:1 support, guidance, and personal assistance in the focus person's home, neighborhood, and community, including helping with hygiene, household chores, and errands. Follow the Individualized Support Plan and policies to maintain the focus person's health and well-being. Coordinate community living services, facilitate person-centered planning, and advocate for necessary resources while representing both the focus person and Creative Support Alternatives in the community. Communicate effectively with the focus person, family, supervisors, and community members.
Required Qualifications
- High school diploma or GED equivalency
- One-year of experience functioning as a team member in group or work setting
- Candidate must reside in California and live within a commutable distance from SDSU at time of hire
- Job offer is contingent upon satisfactory clearance based on background check results (including a criminal record check)
