Job Summary

CRNP, UPMC Family Hospice — Full-Time Advanced Practice Provider responsible for delivering comprehensive patient and family-centered hospice/palliative care. Duties include establishing and maintaining collaborative relationships with physicians and care teams, planning and coordinating the care plan, conducting symptom management visits (in-person and home-based), performing regulatory-compliant virtual/in-person face-to-face visits, and providing on-call coverage within the IPU and community hospice teams. The role emphasizes clinical leadership, independent judgment, communication, multidisciplinary collaboration, quality, safety, and patient/family education across the life span in home and community settings. Requires active Pennsylvania RN license, CRNP certification, and completion of an approved NP program; BSN/MSN preferred; on-call and travel within designated service areas may be involved. Required Qualifications

  • Current state RN license
