Job Summary

Carry a primary caseload providing Critical Time Intervention services to individuals with severe mental illness and a history of homelessness. Develop person-centered care plans, share knowledge of resources, and engage individuals in skill building while offering crisis management. Collaborate with community providers and informal supports to assist recipients in navigating complex systems. Work Monday through Friday from 8:00am to 4:30pm with one late shift per week and evening on-call responsibilities. Full-time hybrid role covering Ontario, Seneca, Yates, Steuben, Schuyler, Tompkins, and Chemung counties.

Required Qualifications

  • High school diploma or equivalency
  • Must have a valid NYS driver's license
  • Full-Time basis
  • Hybrid work model
  • Services in Ontario, Seneca, Yates, Steuben, Schuyler, Tompkins and Chemung counties
  • Monday-Friday, 8:00am-4:30pm
  • One late shift per week (12pm-8:30pm)
  • Some evening on call responsibilities

Desired Qualifications

  • NY State Certified Peer Specialist Certification highly desired
  • 6 months of Health or Human Service experience with severe mental illness and substance use preferred
  • Case Management experience is highly desired
