Job Summary
Carry a primary caseload providing Critical Time Intervention services to individuals with severe mental illness and a history of homelessness. Assess basic health and medical needs within scope of practice, coordinate care with community providers, and deliver rehabilitation, education, and support services. Work to build relationships with key community providers while completing all client visits in the community. The role operates on a Monday-Friday schedule with one late shift per week and includes evening on-call responsibilities. Full-time employees receive a sign-on incentive up to $4,000 alongside comprehensive benefits including health insurance, paid time off, and a 401(k) match.
Required Qualifications
- Must be a licensed practical nurse currently registered by the New York State Education Department
- Basic computer skills
- Must have a valid NYS driver's license
