Critical Time Intervention Licensed Practical Nurse (LPN) (Oneida County)

Carry a primary caseload providing Critical Time Intervention (CTI) services to individuals with severe mental illness transitioning from hospital to community care. Assess basic health and medical needs within scope of practice, coordinate care with community providers, and deliver rehabilitation, education, and support services. Work full-time in a hybrid model throughout Oneida County, creating relationships with key community providers while managing evening on-call responsibilities. A licensed practical nurse registered by the New York State Education Department is required, with experience in substance use disorders preferred.
