Job Summary
Coordinate Critical Time Intervention services for adults with serious mental illness, overseeing outreach to inpatient hospitals, crisis units, and community settings to support up to 30 clients. Direct supervision of 2-3 care managers, including interviewing, hiring, training, and resolving personnel issues while managing daily caseloads and case presentations. Develop coordinated admission and transition plans with providers, review discharge protocols, and ensure compliance with state policies and grant requirements. Facilitate weekly team meetings, monitor documentation quality, and implement corrective action plans based on data collection and quality assurance activities. Maintain fiscal responsibility by reviewing profit and loss analysis and approving service dollar requests.
Required Qualifications
- Current valid license as a Physician issued by NYSED
- Current registration to practice as a physician assistant issued by NYSED
- Current certification to practice as a nurse practitioner issued by NYSED
- Current license as a registered professional nurse issued by NYSED
- Current license as a psychologist issued by NYSED
- Current license as a licensed clinical social worker (LCSW) issued by NYSED
- Current license as a mental health counselor issued by NYSED
- Current license as a marriage and family therapist issued by NYSED
- Current license as a psychoanalyst issued by NYSED
- Current license as a creative arts therapist issued by NYSED
Desired Qualifications
