Job Summary
CTI Coach responsible for direct client contact within the Critical Time Intervention (CTI) model across three phases (Pre-CTI to long-term stability). Duties include rapid engagement of referred individuals, facilitating enrollment, assessing barriers, and coordinating with primary supports and community providers to connect clients to housing, employment, entitlements, and other long-term services. The role emphasizes collaborating with teams to develop comprehensive plans for stability and safety in the community, effective communication in writing and speech, and utilization of provided technologies. Requires a Bachelor’s Degree, valid driver’s license with reliable transportation, COVID-19 vaccination willingness/status, background check, and a supportive, client-centered approach. Location is Rochester, NH with on-site work.”,
Desired Qualifications
- Bachelor’s Degree
- Experience in behavioral health and/or social determinants of health preferred
- Experience with social determinants of health-related programs (housing, employment, food insecurity, entitlements) preferred
- Understanding of NH systems of care, with experience working between systems preferred
- Ability to communicate effectively both orally and in writing
- Ability to utilize technology provided to perform requirements of the job
- Valid driver’s license, proof of automobile liability insurance and reliable transportation
