Job Summary
Conduct comprehensive assessments of client needs, strengths, and goals to develop individualized, short-term support plans focused on stability and transition. Coordinate transitions from hospitals or other settings into the community by connecting people to resources including housing, employment, and healthcare. Collaborate with healthcare providers, social services, and community organizations to advocate for client needs across systems while maintaining accurate, timely, and compliant documentation. Build strong, trusting relationships to encourage engagement in services and monitor progress to adjust plans for successful outcomes. Identify and address barriers to long-term stability, providing education and support to individuals and families during critical transitions.
Required Qualifications
- Bachelor's degree in social work, psychology, or related field
- 2+ years of case management or related experience
- Valid driver's license and reliable transportation
- Flexibility for occasional evenings or weekends as needed
- Working knowledge of Microsoft Office
- In compliance with federal law, all persons hired will be required to verify identity and eligibility to work in the United States and to complete the required employment eligibility verification form upon hire
