Job Summary
Carry a primary caseload providing Critical Time Intervention services to individuals with severe mental illness and a history of homelessness. Develop person-centered care plans, share knowledge of resources, and engage individuals in skill building. Offer crisis management, collaborate with community providers, and assist recipients in navigating complex systems while working with informal supports. Operate in the community throughout Montgomery County on a full-time hybrid basis, Monday through Friday with one late shift weekly and evening on-call responsibilities.
Required Qualifications
- Associates degree
- One year employment experience in the Human Services field
- Equivalent combination of training and experience (substituting for educational and employment requirements)
- Basic computer skills
- Valid NYS driver's license
- Monday-Friday, 8:00am-4:30pm schedule
- One late shift per week (12pm-8:30pm)
- Evening on call responsibilities
- Full-Time basis
- Hybrid work model
- Working in the community throughout Montgomery County
Desired Qualifications
- Bachelor's degree
- Experience and/or training in the care coordination for those with severe mental illness and substance use disorders
