Job Summary
Provide stabilization and support services for consumers during crisis periods, assisting in developing safety plans and connecting individuals to local community resources. Support the treatment team in implementing crisis plans while communicating with stakeholders after each incident. Prepare and submit required documentation, including evaluations of services and records of interventions, ensuring alignment with DHS and agency policies. Collaborate closely with case management and clinical teams to promote consumer wellness and foster community integration. Travel approximately 50% using agency vehicles to multiple locations, including community sites and client homes.
Required Qualifications
- Bachelor's degree in Human Services or a related field
- One year of experience working with the target population and/or providing crisis and mental health services
- Strong organizational skills, ability to manage multiple complex tasks, strong professional communication and team work
- Strong verbal and written communication skills, including the ability to perform record keeping, documentation, prepare formal reports using proper writing skills
- Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors
- Ability to perform computer tasks using MS Windows applications, Agency data base systems, etc.
- Valid Wisconsin driver's license and personal liability insurance
- Ability to use personal and agency vehicles for work related duties according to the agency's insurance liability standards
- Travel approximately 50% using Agency's vehicle(s)
