Job Summary
Deliver best-practice crisis response and stabilization services to Hennepin County families with youth ages 0-18, responding in-home within one hour. Identify resources, screen for trauma exposure, and develop clinical plans with supervisor oversight to connect families with medical, social, and educational supports. Maintain community contacts for seamless referrals, provide complete documentation within timelines, and practice self-care strategies to mitigate secondary traumatic stress. Work weekends and on-call rotation as needed for 24/7 coverage across hospitals, homes, and schools.
Required Qualifications
- Bachelor's degree in a helping field such as Social Work, Counseling or other Human Service field
- 1+ years' experience in crisis assessment and de-escalation, behavioral intervention, and/ or direct work with youth and families
- Valid driver's license and car insurance
- Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges
- Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values
- Must successfully pass a DHS criminal background check
- Weekends required
- Shifts include all hours as program is 24/7
- This is a hybrid position requiring in person, remote, and on the road
Desired Qualifications
- Experience in crisis/mobile/residential or community work with children/youth and families
- Experience working within home and/or community settings
- Experience working in diverse communities and populations
