Job Summary
Coordinate care and provide support to children and youth experiencing crises. Deliver Caring Contacts, reconnect former clients, and coordinate services with internal and external providers. Conduct outreach to disengaged clients, maintain ongoing follow-up, and serve as a liaison to schools and community agencies to ensure integrated, culturally competent support. Maintain accurate records and documentation of interactions, progress, and referrals. The ideal candidate will have crisis management and case-management experience and be skilled in collaboration and communication to support at-risk youth in Delaware.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology, Sociology, or related field
- Two years of experience in case management, preferably in healthcare, mental health services, or crisis intervention programs
- Familiarity with Delaware’s behavioral health systems, state regulations, and community resources for children and families
