Job Summary

Conduct initial and follow-up clinical risk assessments for patients with behavioral health stressors, formulate provisional dispositions, and manage de-escalation interventions independently or within an interdisciplinary team. Complete comprehensive documentation including safety plans, legal admissions paperwork, and departmental reporting while collaborating with nursing, medical providers, and external agencies. Provide family intervention, support, and psychoeducation regarding the system of care and aftercare planning. Assist with pre-certification authorizations and coordinate remote assessments for additional crisis sites as requested.

Required Qualifications

  • Licensed Social Worker (LSW)
  • Licensed Alcohol and Other Drug Counselor (LAC)
  • Ability to conduct and complete initial and follow-up clinical and risk assessments for patients with behavioral health, co-occurring and related behavioral health stressors
  • Ability to formulate provisional dispositions and clinical summaries utilizing/referencing clinical features
  • Ability to complete assessments for substance use concerns and withdrawal, identifying symptoms, patterns and risks
  • Ability to provide precise and accurate clinical summaries/reviews with psychiatric providers to development appropriate clinical dispositions
  • Ability to formulate after-care plans, assisting patients in navigating the system of care and securing needed follow-up clinical appointments
  • Ability to complete de-escalation interventions with patients independently and as part of an interdisciplinary team
  • Ability to complete re-assessments and ongoing clinical interventions for identified treatment targets
  • Ability to facilitate and manage transfer to involuntary, voluntary and CCIS units for ongoing care
  • Ability to complete initial crisis evaluations, risk assessments, safety plans and clinical reevaluations assessments
  • Ability to manage and complete all required departmental reporting documentation and data collection processes
  • Ability to complete and manage all required legal documentation for involuntary, voluntary and pediatric psychiatric admissions
  • Ability to complete all required insurance authorizations and follow-up as needed to obtain coverage for the needed level of care
  • Ability to conduct and document collateral information and reports when clinically indicated
  • Ability to provide family intervention, support and psychoeducation pertaining to the system of care, psychiatric admission, involuntary process and aftercare planning
  • Ability to collaborate as part of an interdisciplinary care team including, nursing, psychiatric providers, social work, medical providers and peer specialists
  • Ability to maintain professional relationships with external professional agencies involved in the patients care
  • Ability to participate in short-term treatment goal planning and aftercare treatment plans
  • Ability to operate as part of an interdisciplinary team in the emergency department providing direct patient interventions and clinical care in coordination with nursing, medical and behavioral health team
  • Ability to assist and complete assessments, authorizations and discharge planning for additional Crisis sites remotely as requested
  • Ability to complete assigned departmental competencies and continuing education requirements as assigned and within the timeframe required
  • Night shift availability
  • Location in or willing to relocate to Atlantic Health Morristown Medical Center, NJ
  • Ability to meet physical requirements as defined by Occupation Safety/OMS
