Job Summary
Coordinate admission and discharge planning for the 23-Hour Crisis facility and Crisis Stabilization site, including screening and evaluation of patients with substance use or mental health concerns, coordinating discharge plans with patients, families, and community agencies, and arranging post-discharge follow-up and referrals to outpatient behavioral health, housing, medical care, or other resources. Collaborates with Peer Support Specialists and Mental Health Clinicians; ensures records align with ASAM standards; participates in regular clinical and staff activities; supports development of safe, timely discharge plans aligned with treatment goals and safety considerations.
Required Qualifications
- Bachelor’s degree or higher (Psychology, Sociology, or Human Services preferred) with minimum of three years of experience relevant to primary responsibilities
- Experience working with individuals in crisis
- Must possess a current First Aid Certificate
- Pre-hire TB test and annual health screening
- Annual influenza vaccine required; exemptions possible with medical/religious approval
- Must have a valid Washington State driver’s license
- Must obtain CPR certificate within 90 days and maintain it
- Must be fully vaccinated for Covid-19 (exemption possible)
Additional Requirements
- Must pass Background Check and Washington State Patrol
- No history or evidence of alcohol or other drug misuse for three years prior to employment and while employed (per policy)
