Job Summary
Coordinate admissions, transfers, and discharge planning in collaboration with the Clinical Director and care team. Serve as a key liaison between clients, families, legal reps, and community service providers to develop and implement person-centered treatment and discharge plans focused on recovery and stability. Identify appropriate post-discharge services such as housing or outpatient care, advocate for clients' rights, and remove barriers to discharge including lack of transportation or insurance. Maintain accurate documentation in compliance with HIPAA and KDADS regulations while supporting the development of client-centered tools like WRAP plans.
Required Qualifications
- Must be at least 21 years old
- Must pass KBI and KDADS background checks
- Must pass drug test
- Maintain valid driver's license and insurable driving record (no substance-related incidents within 5 years)
- Must meet all health screenings, TB testing, and physical clearance requirements
- Experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree
- One year of related work = one year of college
- Ability to handle high-pressure situations and respond to emergencies
Desired Qualifications
- Experience in mental health, crisis intervention, or substance abuse treatment preferred
