Job Summary
Maintain a client caseload by conducting substance use disorder screenings, crisis interventions, and individual, group, or family counseling for mental health and substance use. Assist clients in creating person-centered treatment plans every 90 days while providing linkage to community resources and referrals for crisis management or behavioral health services. Develop comprehensive discharge plans in collaboration with clients, families, and multidisciplinary staff. Complete mandatory electronic health record documentation and urine drug screens within regulatory timelines. Attend weekly staff meetings, treatment teams, and supervision. Collaborate with local agencies including DSS, Probation, and Drug Court. Comply with Office of Mental Health, OASAS, New York State, and Federal regulations. Occasional client home visits required. Full-time, 40 hours per week with variable scheduling.
