Craig: Care Coordinator

Care Coordinator for Home Care in Western Colorado. Manage an assigned caseload of clients, coordinate care with clients, families, caregivers, and community partners, complete intake assessments and required client visits, support caregiver scheduling and coverage, and address client or staff concerns promptly and professionally. Work from home with occasional in-office meetings, visits to client homes, and up to 50% travel across the field. Requires strong communication, organization, and problem-solving skills; able to work independently, in a fast-paced environment, and as part of a team.
