Job Summary

Manage prenatal patient case management including initial, trimester, and post-partum assessments, interventions, referrals, and follow-up services across obstetrics, nutrition, and health education. Implement and integrate CPSP program assessments, psychosocial support, and perinatal education while ensuring compliance with Federal, State, and Los Angeles County standards. Represent the clinic with medical providers and the Los Angeles County Department of Public Health to develop program goals and maintain open communication. Document program eligibility, maintain logs for reporting, and participate in continuous quality improvement plans. This full-time Monday through Friday role requires a reliable vehicle, bilingual communication skills, and CPHW certification.

Required Qualifications

  • High School diploma
  • CPHW Certification
  • Knowledge of women's health prevention
  • Knowledge of OB/GYN family practice programs
  • Knowledge of CPSP
  • Knowledge of Family PACT
  • Knowledge of CHDP programs
  • Knowledge of budget and grant management
  • Knowledge of program evaluation methods
  • Ability to be a team player
  • Ability to relate and communicate well to all cultural and ethnic groups in the community
  • Valid California Driver License
  • Automobile insurance coverage
  • Reliable automobile for use on the job
  • Subject to a criminal background check prior to employment
