Job Summary
Monitor treatment adherence, coordinate referrals for detox and residential care, and assist with health insurance applications and transportation. Manage client risk in collaboration with the treatment team, develop individual service plans, and facilitate weekly recovery skills groups. Provide discharge planning, link clients to ongoing substance abuse and medical treatment, and involve family members and referral sources. Complete all required documentation per protocol and actively participate in site coordinating, technical assistance, and staff meetings. Maintain effective communication with staff and external sources to promote superior customer service.
Required Qualifications
- High School Diploma/GED
- One year supervised counseling experience in substance use disorder treatment or a closely related field
- Proven ability to multitask and meet established deadlines
- Outstanding customer service skills
- Attention to detail
- Valid driver's license
- Access to insured auto
