Counselor

Conduct individual, group, and family counseling sessions while assessing patient needs using DSM criteria and ASAM guidelines. Develop and implement individualized treatment plans, reassess patients, and revise plans as clinically indicated. Collaborate with the multidisciplinary team to make patient care decisions, facilitate crisis interventions, and coordinate support services for patients and families. Document all assessments, referrals, and follow-ups in clinical records while maintaining caseload responsibilities including case management and utilization reviews. Create discharge and continuum-of-care plans to ensure appropriate transitions from admission through follow-up. Maintain an assigned caseload and complete required training in a timely manner.
