Job Summary
Conduct community outreach and screen clients for social determinants of health and 1115 Medicaid waiver eligibility. Assist with benefit applications, navigate to available resources, and make referrals to agencies providing food, legal, and medical services. Provide short-term counseling, advocacy, and coordinate services with partnering organizations. Complete documentation in EPIC and attend required staff meetings. Strengthen relationships with community partners to identify resources for clients. This role supports the network's behavioral health program and serves disenfranchised New Yorkers through eight primary care sites and school-based health centers.
Required Qualifications
- High School Diploma or equivalent
- Knowledge of public benefits
- History of immigrant community engagement
- Ability to effectively communicate with all levels of the organization
- Spanish or Mandarin speaking
