Job Summary

Counselor III provides evidence-based clinical interventions, case management support, and crisis intervention in an adult outpatient integrated care setting. The role emphasizes trauma-informed practice, cultural diversity awareness, and collaboration with a multidisciplinary team that includes medication management and primary care services. Responsibilities include conducting comprehensive behavioral health assessments, developing individualized service plans, maintaining accurate electronic health records, delivering strengths-based case management and motivational interviewing in community and clinic settings, building trusting client relationships to support self-advocacy and goal attainment, coordinating with community partners to promote stability, providing crisis intervention, ensuring compliance with agency policies and regulations, and assisting clients in obtaining medical, housing, and social services. The position requires a Master’s degree in a behavioral science field and credentialing as QMHP or equivalent licensure, with CPR certification via post-hire training. Required Qualifications

  • Master’s degree in behavioral science field from an accredited college or university
  • Demonstrate the competencies to become credentialed as a Qualified Mental Health Professional (QMHP), as outlined in the Oregon Administrative Rulings (OARs); or credentialed through state licensing boards: Board of Licensed Socials workers, Board of Licensed Professional Counselors
  • Certification must be kept current and in compliance with OARs through MHACBO or respective licensing board
  • Ability to conduct a Mental Status assessment and formulate diagnosis according to the most recent DSM
  • Proficient verbal and written communication skills
  • Must understand recovery model, motivational interviewing, illness management and recovery, evidence-based practices, and strength-based case management philosophies and strategies
  • Knowledge of Microsoft Office Suite (e.g., Word & Excel)
