Counselor III - Integrated Team Based Care (Adult Outpatient)

Counselor III provides evidence-based clinical interventions, case management support, and crisis intervention in an adult outpatient integrated care setting. The role emphasizes trauma-informed practice, cultural diversity awareness, and collaboration with a multidisciplinary team that includes medication management and primary care services. Responsibilities include conducting comprehensive behavioral health assessments, developing individualized service plans, maintaining accurate electronic health records, delivering strengths-based case management and motivational interviewing in community and clinic settings, building trusting client relationships to support self-advocacy and goal attainment, coordinating with community partners to promote stability, providing crisis intervention, ensuring compliance with agency policies and regulations, and assisting clients in obtaining medical, housing, and social services. The position requires a Master’s degree in a behavioral science field and credentialing as QMHP or equivalent licensure, with CPR certification via post-hire training.
