Job Summary
Conduct psychosocial, behavioral, and neuropsychological assessments of program participants and write accurate, timely professional reports. Evaluate individual needs to develop treatment plans aligned with participant goals, mental health status, and care requirements. Participate in daily interdisciplinary team meetings to support integrated care management and assist in care coordination between assisted living and skilled facilities. Collect data on health services and participant outcomes, respond to crisis situations for previously active participants, and maintain thorough progress notes within specified timeframes. Coordinate ongoing training programs for internal and external customers on behavioral health issues. Requires a Licensed Professional Counselor or Licensed Clinical Social Worker in Colorado with one year of experience working with the frail/elderly population.
Required Qualifications
- Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) in the state of Colorado
- A minimum of one-year experience working with the frail/elderly population
- Experience with severe and persistent mental illness and personality disorders
- Thorough working knowledge of the DSM-V/ICD-10 codes
- Experience working in a non-restraint, person-centered environment
Desired Qualifications
- CACII
- Experience and training in Dialectic Behavior Therapy, Substance Abuse, and trauma-informed counseling
- Experience charting in and electronic health record
