COTA

Assist the occupational therapist in evaluating patient function levels, preparing clinical and progress notes, and developing treatment plans. Advise and consult with families and agency personnel, while observing, recording, and reporting changes in patient conditions. Educate patients and families on equipment use and home therapy programs, and assist with rehabilitation according to the plan of care. Participate in case conferences and in-services, communicate effectively with all care providers, and ensure quality care. Confirm weekly scheduling of visits with the occupational therapist to coordinate necessary visits with other personnel. This part-time role in our skilled home care division requires current Michigan COTA licensure, one year of home care experience, CPR certification, and a valid Michigan driver's license.
