Assess patient function levels, prepare clinical and progress notes, and develop treatment plans under licensed supervision. Advise families and agency personnel, observe and report condition changes, and educate clients on equipment and home therapy programs. Assist with rehabilitation according to the care plan, participate in case conferences, and coordinate weekly visit scheduling. Maintain clear communication with all care providers to ensure quality care. This part-time role offers weekly pay, mileage reimbursement, and paid training within Interim HealthCare's skilled home care division.
