Job Summary
Assess patient function levels, prepare clinical and progress notes, and develop treatment plans under licensed supervision. Advise families and agency personnel, record condition changes, and educate clients on equipment and home therapy programs. Assist with rehabilitation per the care plan, participate in case conferences, and coordinate weekly visit schedules. Maintain positive rapport with clients and communicate effectively across the interdisciplinary team.
Required Qualifications
- Current licensure as a Certified Occupational Therapy Assistant (COTA) in the state of Michigan
- CPR certification
- Current Michigan driver's license
- Reliable transportation
- Proof of car insurance
Desired Qualifications
- A minimum of one year in a home care setting preferred
