Job Summary
Assist the occupational therapist in evaluating patient function levels, preparing clinical and progress notes, and developing treatment plans. Advise and consult with families and agency personnel, while observing and reporting changes in patient conditions. Educate patients and families on equipment use and home therapy programs, and assist with rehabilitation according to the care plan. Participate in case conferences, in-services, and interdisciplinary team communications to ensure quality care. Confirm weekly visit scheduling with the therapist and coordinate necessary visits with other personnel. Requires current Michigan COTA licensure, CPR certification, and a valid driver's license.
Required Qualifications
- Current licensure as a Certified Occupational Therapy Assistant (COTA) in the state of Michigan
- CPR certification
- Current Michigan driver's license
- Reliable transportation
- Proof of car insurance
Desired Qualifications
- A minimum of one year in a home care setting preferred
