Cornerstone Leader - Memory Support

Champion the Cornerstone Program's guiding principles across the memory care neighborhood, guiding families through the Lifestory process to shape personalized care. Help hire, train, and mentor team members while partnering with nursing, therapy, culinary, and activities staff to build a program meeting residents at every stage of dementia. Lead the move-in experience from pre-admission through the first days, facilitate monthly care meetings with families and the interdisciplinary team, and conduct regular environmental rounds to ensure safety and purpose. Ensure compliance with state and federal regulations and build partnerships with outside organizations that enrich resident experiences. Requires a degree or relevant experience in memory care, two-plus years working with cognitive impairment, and a certified Dementia Practitioner certification.
