Job Summary
The HCBS Provider at FREE is responsible for delivering community-based non-clinical rehabilitation services, assessing individual needs, and crafting specific, measurable plans to achieve set goals. This position requires effective communication with both clients and team members, adherence to quality documentation standards, and accountability for a caseload of approximately 20 individuals. The role also involves field-based treatment, maintaining confidentiality, collaborating within an interdisciplinary team, and fulfilling various documentation and administrative tasks. Skills in communication, interpersonal relations, and organization are essential.
Required Qualifications
- Bachelor’s degree in mental health or social services related field required
- One year of experience providing individual, group, or family therapy to individuals with mental illness is preferred
- Valid driver’s license and meets all the criteria to be a valid driver for the agency
- Willingness to travel a must
