Job Summary
Conduct 1:1 skilled nursing, wound care consultations, and client assessments in the comfort of patients' homes. Deliver intervention and supportive care including medication administration, treatments, and comfort care while educating families and monitoring conditions. Manage care from hospital discharge to independent living with oversight of wound care plans. Maintain a calm, reassuring presence to advocate for client needs and ensure safety standards. Schedule is Monday through Friday only, excluding weekends and holidays.
Required Qualifications
- Active and current license to practice as a professional registered nurse (RN) in the state of PA
- Wound, Ostomy and Continence Nurse (WOCN) certification
- At least one year of nursing experience
- Ability to travel to cases as assigned
- Monday - Friday position, NO Weekends and NO Holidays
