Nurse Practitioner (APRN-CNP) sought to provide in-home primary care to homebound Medicare/Medicaid patients across Las Vegas and surrounding areas. Role focuses on house visits with telehealth urgent care as needed, and offers flexible scheduling (full-time or 2–3 fixed days per week). Responsibilities include conducting comprehensive home visits, evaluating and managing chronic/acute conditions, medication reconciliation and lab reviews, coordinating with care teams, responding to urgent care needs via telehealth, ensuring accurate documentation for compliance and billing, and collaborating with care coordinators and specialists. Requires NV APRN-CNP license, Spanish proficiency, Medicare and Nevada Medicaid credentialing (or eligibility), at least 1 year of relevant experience, reliable transportation, comfort with telehealth tools; bilingualism preferred but not required. Per-visit or daily pay structure with in-person visits and telehealth as needed.
