Job Summary
Assess the psychosocial status of patients and families regarding terminal illness and environment, communicating findings to the registered nurse and interdisciplinary group. Conduct social evaluations covering family dynamics, caregiver abilities, and risks for suicide, neglect, or abuse to plan interventions and provide crisis support. Educate interdisciplinary teams on cultural needs, assist in preparing advanced directives, and serve as a liaison between patients and community agencies. Maintain clinical records, participate in quality assessment teams, and assist in planning funeral arrangements and financial/legal decisions. Requires a master's or bachelor's degree in social work, one year of healthcare experience, current CPR certification, and a licensed driver's license. No benefits are provided for PRN positions.
Required Qualifications
- A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education
- A graduate of a bachelor's program in social work accredited by the Council on Social Work Education
- Meets personnel qualifications stated in §418.114(b)(3)
- Minimum of one (1) years of experience in health care
- Possesses and maintains current CPR Certification
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state or organization requirements
- Must have an automobile that is in good working order
- Must be able to occasionally lift and/or move up to 50 pounds
Desired Qualifications
