Job Summary

Provide patient-centered home health physical therapy services on a PRN basis. Responsibilities include evaluating patients to develop individualized therapy plans, implementing treatment programs to improve mobility, strength, flexibility and functional performance, utilizing therapeutic exercises, manual therapy, modalities, and assistive devices, educating patients and families on home exercise programs and strategies to support recovery, documenting progress and coordinating care with physicians and the healthcare team, and ensuring compliance with regulatory standards. Requires a valid PT license and 1+ year of PT experience; strong communication, ability to manage multiple priorities, empathic patient interaction, and ability to lift up to 50 lbs. Schedule offers flexible availability across weekdays and weekends; mileage reimbursement provided.

Required Qualifications

  • Valid Physical Therapy license in the state where work is performed
  • 1 or more years of physical therapy experience, preferably in home health or similar setting
  • Excellent communication and interpersonal skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Ability to build rapport with patients
  • Physical ability to lift and transfer patients up to 50lbs
  • Capability to perform physical tasks (bending, standing, walking, stairs)
  • Capacity to work in various home environments
  • Maintain regular attendance and timeliness as designated
