Job Summary
Direct and coordinate patient care for multiple home locations, ensuring continuity, quality, and safety in compliance with state and federal regulations. Supervise subordinates to ensure adherence to the Plan of Care, while monitoring the QAPI Program and identifying system problems for reporting. Facilitate communication between the site, parent office, interdisciplinary groups, and referring hospice providers. Provide patient care as needed and participate in after-hours on-call rotations for all locations. Travel locally to various sites, with requirements for prolonged standing, lifting up to fifty pounds, and working under stressful conditions.
Required Qualifications
- Registered professional nurse
- Graduate from an accredited diploma, associate, or baccalaureate nursing program
- Active, unencumbered state license as a registered nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state
- Current driver's license
- One year of experience as a registered nurse in a clinical care setting
- Nursing skills as defined as generally accepted standards of practice
- Excellent interpersonal skills and ability to communicate effectively
- Demonstrates proven decision-making skills
- Must read, write, and comprehend English
- Reliable transportation
- Valid auto liability insurance
- Ability to travel locally
