Job Summary
Maintain a caseload of 30-40 clients with intellectual or developmental disabilities by advocating for their needs, facilitating person-centered planning meetings, and conducting required assessments per DDA regulations. Research and implement community services including medical, social, vocational, and legal supports while monitoring service provision through regular client visits and information gathering. Assist with client enrollment activities, respond to provider-generated incident reports, and document client services accurately within Maryland's Long-Term Services and Supports system. Visit clients in various settings at least four times annually depending on coordination intensity, negotiate interdisciplinary team disagreements, and ensure continuity of comprehensive care.
Required Qualifications
- Bachelor's degree in a human service focus, or an Associate's degree in a human service focus and two years of human service experience, or seven years of specific human service work experience
- Experience in coordinating services for individuals in Medicaid or waiver programs or coordination services for individuals with Intellectual or Developmental disabilities
- Must have access to a reliable automobile and a valid driver's license
- Excellent communication skills-writing and speaking
- Computer skills including Word, Excel, and PowerPoint
- Ability to manage complicated work processes and multi-task
- Negotiation and conflict management and crisis management
- Effective advocacy skills and problem solving
- Proven ability to establish and maintain working relationships
- Person- and family-centered care
- Self-determination planning
- Community resources and service delivery systems (i.e., generic, local, State, and federal programs, Medicaid, mental health, substance abuse, social services, juvenile justice, vocational rehabilitation, and corrections)
- COMAR Regulations governing services for individuals with Intellectual/ Developmental disabilities
- Strong belief in the opportunities for people with disabilities to fully participate in the community and a willingness to work toward individuals� identified goals
Desired Qualifications
- Experience in coordinating services for individuals in Medicaid or waiver programs or coordination services for individuals with Intellectual or Developmental disabilities is strongly preferred
