Job Summary
Coordinate, implement, and evaluate the home care needs of patients recovering from illness, injury, or surgery at home. Create initial comprehensive interdisciplinary patient-centered home health plans of care and update them in collaboration with physicians. Review patient insurance information, coordinate individual care plan activities, and share pertinent findings with family and professional caregivers. Provide advanced clinical services including patient assessment, medication management, and patient/family education while supporting agency financial goals. This role requires a current NYS RN license, Bachelor's degree in nursing, and minimum one year of acute medical-surgical nursing experience. CHHA experience and bilingual English/Spanish skills are preferred. A driver's license and car are required.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team

- Bachelor's degree in nursing preferred
- CHHA experience preferred
