Job Summary
Coordinate, implement, and evaluate the home care needs of patients recovering from illness, injury, or surgery at home. Create initial comprehensive, interdisciplinary patient-centered home health plans of care and update them in collaboration with physicians. Manage medication, conduct patient assessments, and provide education to patients and families. Review insurance information, coordinate individual care plan activities, and share pertinent findings with professional caregivers. Minimize rehospitalization risks while supporting agency financial goals through expert clinical assessment and independent problem-solving. Requires current NYS RN license, Bachelor's degree, acute medical-surgical experience, and bilingual English/Spanish proficiency.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
- Bilingual English / Spanish preferred
