Job Summary
Coordinate comprehensive, interdisciplinary home health plans of care for patients recovering from illness, injury, or surgery, conducting patient assessments, managing medications, and providing family education. Oversee the implementation and continuous evaluation of care while reviewing insurance information and ensuring findings are shared with professional caregivers. Case manage assigned patients to achieve positive clinical outcomes and minimize rehospitalization risks, supporting agency financial goals through expert community health nursing knowledge and independent problem-solving. Requires current NYS RN license, Bachelor's degree, acute medical-surgical experience, and CHHA preference. Driver's license and car required.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
- Excellent communication, documentation and time management capabilities
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
