Job Summary
Coordinate and implement comprehensive home health plans of care for patients recovering from illness, injury, or surgery. Conduct patient assessments, manage medications, and provide education to families and professional caregivers. Review insurance information, update care plans in collaboration with physicians, and ensure findings are shared with all stakeholders. Case manage services to achieve positive clinical outcomes and minimize rehospitalization risks while supporting agency financial goals. Requires current NYS RN license, acute medical-surgical experience, and a driver's license.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
