Coordinate and oversee the home care needs of patients recovering from illness, injury, or surgery by creating initial interdisciplinary care plans and updating them in collaboration with physicians. Conduct patient assessments, manage medications, and provide education to families and professional caregivers while reviewing insurance information and coordinating activities for each individual care plan. Share pertinent findings with family members and ensure all clinical findings are accurately documented. Effectively case manage services to achieve positive clinical outcomes and minimize rehospitalization risks while supporting the agency's financial goals. Maintain current NYS RN licensure and demonstrate expert knowledge in community health nursing, acute medical-surgical practice, and bilingual English/Spanian communication.
