Job Summary
Coordinate and implement comprehensive home care plans for patients recovering from illness, injury, or surgery, conducting patient assessments, managing medications, and providing family education. Review insurance information, update care plans in collaboration with physicians, and ensure pertinent findings are shared with professional caregivers. Effectively case manage services to achieve positive clinical outcomes and minimize rehospitalization risks while supporting the agency's financial goals. Demonstrate expert knowledge of community health nursing, including acute medical-surgical experience and CHHA proficiency, utilizing strong independent problem-solving and communication skills. Requires current NYS RN license, Bachelor's degree preferred, driver's license, and car.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
